coaches and carers are free and clear of all responsibility whatsoever in the above
mentioned activity for any loss, injury or illness during the applicant’s participation
in the academy. In the event of an injury or illness to my child I hereby give consent
in arranging any medical treatment that may be required.

INSURANCE:
We encourage all participants to review their own individual needs and circumstances and
where appropriate take out Private Health, Life and Income Protection Insurance. It is
an Individual’s responsibility to ensure that he/she has adequate insurance cover for
his/her needs.
FEES:
If I decide to cancel this registration I must give written notice one month prior and
understand that all training sessions and player kit must be paid for and I must pay
20% cancellation fee.
Installment plan: Payment by installment plan must be
paid before or on the day of due date. A 1% per day will be charged after 7 days
if due payment has not been made within 7 days.
